Our Programs

Premier’s residential community service programs are developed in accordance with and funded through the Oklahoma Department of Human Services, Developmental Disabilities Services Division’s Community Waiver Program. The Community Waiver Program includes a comprehensive array of services including residential, employment and habilitation services and support - appropriate to the individual’s needs. A plan of care is developed annually for each individual receiving services through the Community Waiver. Using a person-centered planning approach, the individual’s team of interdisciplinary professionals assess the needs of the individual and develops a plan to address those needs.

Premier’s Services include: In-Home Support, Agency Companion, Daily Living Support and Supported Living. Detailed descriptions of each program are below:

Family Dinner

Agency Companion Services 

This program is a contractor model living arrangement where the Companion (caregiver) is certified through OKDHS/DDS to provide support services to one individual in the Companions personal home.

The Companion is an independent contractor who contracts directly with Premier to provide the individual’s support needs in a shared living arrangement which fosters the experience of a family setting.

 

In this living arrangement, the individual served contributes to the household living expenses through a room and board payment established by the interdisciplinary team.
